Your monthly payment of $129 goes into your personal bank to use on any treatment at any time!
Eligibility & Membership
Open to individuals 18+ who agree to these terms. Membership is $129/month with a minimum 6-month commitment. Benefits begin on the date of payment processing, subsequent payments will be processed on the 10th of each following month.
Monthly Credit
Your $129 monthly credit is banked to your account for treatments or retail for the year of enrolling (i.e. you must use your credit within 365 days of the date of your last month of membership enrollment).
Wrinkle Relaxer Discount
Lock in $13/unit for Wrinkle Relaxers for 365 days at the start of membership. This is non-transferable and cannot be combined with other Refreshed promotions, however your bank dollar amount can be used for any promotion. Vendor reward (Alle/Aspire/Xperience) points can be used.
Complimentary Services
1 complimentary 30-minute Refresh facial every 6 months of active membership.
Facials cannot be exchanged for other services or cash and expire if not used within the 6-month period of membership initiation.
Cancellation
Minimum 6-month commitment. After this period, membership will automatically renew monthly unless canceled 30 days prior to the next billing cycle.
Cancellations can be submitted via email to [hello@refreshedmd.com] or in person at our studio. Membership benefits, including unused credits, will expire upon cancellation unless redeemed prior to the termination of the membership
No refunds for prior payments.
Refreshed M.D. reserves the right to terminate memberships for violation of these terms, inappropriate conduct, or failure to disclose necessary medical information to provide treatment. Terminations will be communicated by email and certified letter. Unused credits will be forfeited unless otherwise stated.
VIP Access
Members enjoy early access to exclusive offers, events, and promotions. Participation is subject to availability.
Liability
Members agree to disclose all relevant medical conditions, allergies, and medications prior to receiving treatment. Failure to disclose such information releases Refreshed M.D. from liability for any adverse reactions or outcomes. Refreshed M.D. adheres to all applicable federal, state and local laws, including privacy regulations, to ensure the confidentiality and proper handling of your medical information. Any disclosed information will be used solely to provide safe and effective treatments.
Transferability
Membership benefits and account credits are non-transferable and can only be used by the registered member.
Missed Payments
If your payment fails to process, your membership benefits will be temporarily suspended until the outstanding payment is resolved. Refreshed M.D. reserves the right to charge a $50 late fee for payments that are more than 10 days overdue. Memberships with unresolved payment issues for over 30 days may be canceled at the discretion of Refreshed MD.
Changes
Refreshed M.D. may modify these terms at any time. Members will be notified of any changes via email with at least 30 days in advance.
Account Access and Balance Inquiry
Members can check their account balance and view their membership benefits at any time by logging into their online account via BLVD or by contacting Refreshed M.D. directly via email [hello@refreshedmd.com]. It is the member’s responsibility to ensure their contact information is up to date to facilitate access to account details.
Privacy Policy
All personal information collected from members, including payment and medical details, will be stored securely in compliance with applicable data protection laws, such as HIPAA. Refreshed M.D. will not share personal information with third parties without your consent. For more details, please review our Privacy Policy at BLVD.
Dispute Resolution
Any disputes arising from this membership agreement shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association [AAA]. By agreeing to these terms, you waive the right to file a lawsuit in court, except for claims that can be handled in small claims court.
